New Client Forms
We are in the process of digitally creating our forms. Once complete, clients will be able to visit this section of the site and download any applicable form. Once downloaded, clients can print the forms, review them, and fill them out prior to coming in for their first massage. This will save the client time that they would have otherwise had to spend at the office filling out the forms.
